Procedure for Handling Disclosure Requests

  1. Verify the identity and authority of the requesting party.
  2. For non-emergency situations, consult with SAFR command before disclosing any information.
  3. Document all requests for information and the response provided.
  4. When in doubt, err on the side of patient privacy and consult with command.

Training and Compliance

All SAFR personnel will receive regular training on medical privacy laws and this policy. Violations of this policy may result in disciplinary action, up to and including termination.
